This article explains how to add sections to the top and bottom of the inner frame of contextual help.

Append/prepend vs. custom header/footer
The contextual help dialog has two separate areas: (1) The inner dialog and (2) the outer frame.
The outer frame is highlighted in blue above and is above and below the inner dialog. Custom header/footer for contextual help targets the outer frame whereas the append/prepend adds to the top and bottom of the inner dialog as it relates to the article being shown.
Here is an example of append/prepend where Top of the Page and Bottom of the Page was added to the top and bottom of the inner dialog:
